
In order to keep the student-athletes of Spalding University on the field, court, course, etc., we must have a complete medical history. We do this so we can be prepared to treat them should an injury occur. The policies of Spalding University and the NCAA also require us to have complete insurance information on file. Each student-athlete must have his or her own primary insurance in order to participate in any aspect of intercollegiate athletics.
ALL STUDENT-ATHLETES must have a physical performed by their family physician or by a physician or nurse practitioner every year. The physical needs to be done prior to reporting to campus in the fall. This physical
MUST BE recorded on Spalding University Athletic Training forms which are listed below.
